Laparoscopically Removal of Large Fibroid with Uterus

A 42-year-old female was admitted with complaints of heaviness in the lower abdomen and swelling over the lower abdomen persisting for 6 to 8 months. The patient did not report any disturbances in menstruation, and her past medical history and family history were not significant.

On examination, the patient was in a stable general condition with Normal vital signs. Abdominal examination revealed a mass of approximately size of a 30-week pregnancy arising from the pelvis and occupying almost more than half of the abdomen. The mass had a firm consistency and was centrally located, non-tender, and exhibited mobility. An ultrasound and MRI Pelvis examination revealed a large subserosal fibroid measuring 18X20 cm with degeneration. Both ovaries appeared normal. The patient underwent an Endometrial Sampling Procedure, and the histopathology results confirmed a benign pathology.

Subsequently, a total laparoscopic hysterectomy with bilateral salpingectomy was performed by Dr. Ravinder Kaur Khurana, Sr. Consultant & Head – Unit II (Obstetrics & Gynaecology), Metro Hospital, Faridabad. The laparoscopic removal of such a large fibroid posed significant challenges. The fibroid was morcellated abdominally and vaginally. During the process of morcellation a portion of the morcellator (tenaculum) broken off, necessitating the removal of the remaining fibroid vaginally. More than 50 percent of the fibroid was removed vaginally. Following the hysterectomy, the patient’s condition is stabilized, and she is currently recovering well.
