Successful ABO incompatible Renal Transplant in a Patient with End-Stage Renal Disease

A 27-year-old male patient with end-stage renal disease (EDRD) was treated at the Metro Hospital, Faridabad. He had been. undergoing maintenance hemodialysis for the past year and the cause of his renal failure was unknown. The patient presented for a live-related renal transplant with his mother as the donor. Despite a mismatch in their blood groups and only a 3/6 HLA match, the transplant was performed successfully.

The patient’s Anti-B titres were evaluated using erythrocyte magnetized technology, which showed a large variation in IgG and IgM titres, with IgM at 1:128 and IgG at 1:4. The significance of this high IgM titre was unclear, as many transplant centers across the world only consider IgG titres as significant. He underwent daily plasmapheresis to remove IgM Antibodies, as IgM Antibodies are difficult to clear.

The patient underwent a kidney transplant, with a successful surgery and peri-transplant period. He was discharged on the seventh post- operative day (POD) with a baseline serum creatinine of 1.2 mg/dl
and continued on triple-drug immunosuppression.

The surgery was performed by Dr. Ritesh Mongha and team. Donor had Laparoscopic Nephrectomy done and was discharged in 3 days. Recipient was discharged on 7th day with normal Kidney functions.

Five months after the transplant, the patient was closely monitored and had stable graft function, with a serum creatinine level of 1.2-1.3 mg/dl. The patient’s successful kidney transplant highlights the importance of of utilizing ut innovative medical technology and expertise to treat patients with ESRD, providing them new lease of life.
